Templates and Guidance for Proposal Submissions
Not all competitions require all of these forms. Refer to the specific RFP/RFA you are responding to for exactly what should be included with your submission.

All projects require a 90-4 form for each year of funding plus a separate narrative. Multi-year projects should ensure their form is expanded to include a 90-4 for the entire project (i.e., if requesting support for a 2-year project, provide a 90-4 for Year 1, a 90-4 for Year 2, and a 90-4 for the total funding request over both years). Unless otherwise noted in the RFP/RFA, the budget must be approved by your institution’s Office of Sponsored Programs or equivalent before submission.
Typically, all applications require this form. This document is meant to be reviewed independent of the proposal application. Complete all questions to the best of your ability. Throughout the document, consider both work in the environment proper as well as materials, procedures, situations, or activities that may be hazardous to personnel (whether or not they are directly related to a particular phase of the proposed project). Be sure to include in the appropriate place an outline or precautions to be exercised to avoid or mitigate the effects of such hazards.
Purdue University Entity Profile (Fillable PDF). This form is not required for applicants from Purdue University or Purdue University Northwest. It should be completed by the office of sponsored programs or equivalent.

Guidance
Sea Grant General Application Guidance (from National Sea Grant Office) All applications that IISG recommends for funding are submitted to the National Sea Grant Office (NSGO) and NOAA for final approvals. The NSGO has prepared guidance to assist applicants to Sea Grant competitions. Not all information in the NSGO guidance will apply to the RFP/RFA you are applying to.
